Dear Parents/Guardians,
In accordance with Senate Bill 12, all Texas public schools are now required to obtain written parental consent before providing any school health-related services to students.
Attached is the 2025–2026 School Health Services Consent Form for your review and completion. This form allows you to either consent to or decline school health services (e.g., first aid, medication administration, vision and hearing screenings) for your child.
To complete the form online, please use the following link: S-TISD Health Services Form.
It is important to complete and submit this form as soon as possible. Please note that if you choose to decline services, no health services—including basic first aid—can be provided to your child during the school day. The only exception is in the event of a life-threatening emergency requiring immediate intervention, such as CPR.
Your timely response helps ensure we can provide appropriate care for your child while maintaining compliance with state laws for the 2025–2026 school year.
